When chronic kidney disease becomes severe, you will be enrolled in the Nierzorg+ (KidneyCare+) programme. At this stage, the kidney filtration rate (eGFR) is less than 25 mL/min.

The Nierzorg+ nurses provide information about kidney failure and assist you in choosing the treatment option that best suits your lifestyle. When kidney replacement therapy becomes necessary, they ensure you receive the necessary information and support.

The Nierzorg+ nurse will contact you to schedule an initial meeting. Your family or close friends are welcome to join this meeting.

The first meeting with the Nierzorg+ nurse lasts about an hour and a half.
Topics covered include:

  • Introduction to the Nierzorg+ nurse
  • Information on what kidney failure is and how to manage it
  • Information on possible treatments to replace kidney function

  • Brochures to take home for further reading
  • Sometimes, you may also meet a member of the social services team

If needed, you will be referred to a dietitian or psychologist. After the initial comprehensive meeting, the Nierzorg+ nurse will follow up with you at each nephrologist consultation, ensuring you have a consistent point of contact for any questions.
