In remote care for heart failure, the hospital, your general practitioner and other healthcare providers (such as a home nurse) work together. This ensures that your care is well coordinated, both in the hospital and at home.
At home, you measure several parameters every day, such as your weight, blood pressure and heart rate. The necessary devices are provided by the hospital. In addition, you can report any symptoms via an app, such as shortness of breath or swelling of the legs. All this data is automatically transmitted to the hospital.
The care team closely monitors your data and contacts you if something deviates. This allows changes in your health to be detected more quickly and can sometimes prevent or shorten a hospital admission
