TaTME (The transanal total mesorectal excision of the rectum)

The TaTME (transanal total mesorectal excision) procedure consists of two parts.

The first part of the operation is transanal (via the anus). In this procedure, the rectum is closed off surgically, incised and then detached from the surrounding structures (vagina or prostate, pelvic muscles and sacrum). This is done via keyhole surgery and with the help of a specially designed anal port (Gelpoint).

The second part consists of keyhole surgery, in which the colon is detached from the stomach and the wall of the abdominal cavity. In this way, we obtain enough length to make a new connection and ensure correct lymph node clearance.

Transanal and keyhole surgery can be performed together or one after the other. The new connection between the anus and the colon is prone to leaks. This is due to the anatomical location, possible blood supply disorders and the preceding radiation, if any.

This is why a temporary loop ileostomy is almost always performed. As a result, there is no stool passage along the operated part. This will allow it to heal better. The ileostoma can be removed after 8 weeks or after any chemotherapy. This will require a second operation, which restores the normal stool trajectory.
